Before appointment:

  • Please fill out New Patient Forms through our secure patient portal before arriving to your appointment.

  • We will review a “COVID-19 Screening Questionnaire” when making an appointment and again upon arrival.

  • Appointments will be managed to allow for physical distancing between patients. Unfortunately, this may mean fewer options for appointment times.

  • We ask that you please do not bring others along to your dental appointment unless special arrangements are made prior to your appointment.

At appointment:

  • Your temperature will be taken with a contactless thermometer before entering treatment rooms.

  • Please bring a face covering to wear in the reception area upon arriving and exiting. If you do not have one, a mask will be provided to you.

  • We will provide hand sanitizer for use before and after touching items in the office.

  • Please dress warmly as UV/Ionizing filters are being used in the office and windows will be open.

After appointment:

  • Please contact our office if you experience COVID-19 symptoms or are diagnosed with the disease within 14 days after your appointment. Please keep us safe!
